New Patient Registration

Please collect a registration pack from reception. You will need your NHS number to register. The registration process will require you to prove that you are living in the practice area, as well as your ID, by producing a passport photograph, or photo ID driver's licence. Please note, you will not normally be offered an appointment with a doctor at registration.

  • All new patients must attend an appointment for a new patient health check.
  • If you miss an appointment to register, you may not be accepted on to the practice list.
  • If you miss an appointment for a new patient health this may affect your registration with the practice.

On occasions we may decline to accept new patients. This will normally be because they deem their list to be full and to take on more patients would be detrimental to those already registered. If you are told that you cannot register, an explanation will be given.

Expressing a Preference

You may express a preference to be seen by a particular health professional. We will endeavour to meet your requirements but often that is not possible. In that event we would explain the reasons why.
